INFORMED CONSENT FORM
My name is Dr AKINOLA T.F. I am a Senior Registrar in the Family Medicine department of the University College Hospital, Ibadan. I am interviewing elderly having hypertension coming to the Chief Tony Anenih Geriatric Centre (CTAGC) of UCH, Ibadan for medical consultation. I want to find out the relationship between family support, psychological well-being and blood pressure control. I will need to ask you some questions, which will be about your health and the hypertension as well as some personal data. Please note that your answers will be kept confidential. The information you and other people give me will help the doctors in the Family Medicine department manage you better.
During this exercise, medical examination will be carried out on you to determine your state of health. Also, some specimen of your urine will be collected and analyzed for protein and glucose. Your honest answers to the questions will be greatly appreciated. You are free to refuse to take part in this program. You have the right to withdraw at any given time if you choose to and this will not affect the quality or standard of care you will receive subsequently.
